Two people walk into my clinic with the same complaint. One leaves with a formula full of warming herbs. The other leaves with almost the opposite. Both get better.

That is the part of this medicine that is hardest to explain in an article and easiest to show. So instead of explaining it again, here is the actual first move I make with a new patient — the sorting question underneath the intake. Eight questions, about two minutes. What just happened

You answered questions about temperature, sleep timing, thirst, and how stress lands in your body — and almost none about your actual complaint. That is deliberate. Those peripheral details are what separate one presentation from another, and they are why a bottle labeled for your symptom so often misses.

Eight questions is a sketch. At a first visit I ask about forty, then check your pulse at three positions on each wrist and look at the shape, color, and coating of your tongue. Those last two are not decorative. They routinely move a patient from the pattern they thought they had to the one they actually have, and the formula changes accordingly.

The four patterns, in plain language

These are working descriptions, not textbook categories — the versions I use when explaining to a patient what I am seeing. Most people are a blend, with one dominant.

Running on empty

Cold hands, heavy fatigue after meals, bloating that feels like weight, loose stools, low voice, worse in the morning.

There is not enough fuel in the system to run digestion properly. Food sits. Energy sags after eating instead of rising. These patients are often eating carefully and exercising hard, which makes it worse — the body needs building up, not further stripping down. Warming, strengthening formulas. Cold smoothies and raw salads tend to backfire here.

Stuck under pressure

Bloating and bowel changes that track stress, tight ribs and shoulders, frequent sighing, symptoms that come and go with no dietary logic.

Nothing is deficient — things simply are not moving. This is the most common pattern I see in working adults in the Portland metro, and the one most likely to be told "your tests are normal." Turned up too high

Heartburn or burning, irritability and a short fuse, thirst for cold drinks, trouble falling asleep, headaches, red face, kicking off the covers.

The system is over-heated and over-activated. Warming tonics — the ginger, cinnamon, and ginseng products people reach for — usually make this pattern worse, which is a common reason someone concludes that herbs "don't agree" with them. Clearing and calming formulas instead.

Burned down to the wick

Wired but exhausted, dry mouth and eyes, night sweats, waking at 3–5 AM and staying up, hard dry stools, worse late at night.

Long-run depletion, often after years of overwork, illness, or hormonal change. Stimulants and strong workouts keep it going and deepen the hole. Quick Answers

Can I really identify my pattern from a quiz?

Not definitively. A self-check narrows the field and shows you the kind of reasoning involved, but a pattern is confirmed in person through pulse and tongue evaluation and a full history. Patients are frequently reclassified at their first visit.

Why do two people with the same symptom get different herbs?

Because the formula is written for the pattern underneath the symptom, not the symptom itself. Bloating from depletion and bloating from heat call for nearly opposite herbs, which is why one mass-produced supplement helps some buyers and does nothing for others.

What if I scored evenly across two patterns?

That is common and clinically normal. Most people present as a blend with one dominant pattern, and formulas are routinely written to address a primary pattern while accounting for a secondary one.

Do I need acupuncture, or can I get herbs alone?

Herbal care can stand on its own, and many patients see Dr. Sohn for formulas only. The initial visit is $175 including the consultation, and subsequent custom herbal formulas are $175 per 7-day supply.

Is it safe alongside my prescription medication?

Bring your complete medication and supplement list to the consultation so Dr. Sohn can review it before prescribing. Formulas are designed to complement the care you are already receiving, and any changes to prescription medication should be made by your prescribing physician.
